ILP Participation of Youth in Placement
The ILP Participation of Youth in Placement form can be used to gather information for your youth’s social worker to enter into CMS as a Deliverable.
There are helpful definitions with this form that will help you make sure the deliverable counts.
This helps us acknowledge the work being done when the state pulls this information in the 6-month reporting period.
