Arranges all service delivery to ILP eligible youth.
Collaborates with staff and providers to ensure quality of ongoing services.
Collaborates with youth, social workers, care providers, and other service providers to ensure the provisions of services and activities so that the youth’s goals are achieved.
Assists with ILP event planning and is a resource for locating education or employment resources.
Monitors the performance and outcome data for ILP services provided.
What does the Social Worker II (SW) Do?
The Social Worker II:
Completes eligibility checks and referrals (i.e., Independent Living Program (ILP), Aftercare, Transitional Housing Placement Plus (THP+), and Extended Foster Care (EFC))
Manages ILP-eligible closed cases (i.e., Adoption, Kinship Guardianship Assistance Program (Kin-GAP), and Guardianship)
Serves as a Children and Family Services (CFS) liaison for Transitional Age Youth (TAY) Centers
Completes ILP planning tasks pertaining to upcoming events and/or workshops
What are Peer and Family Assistants (PFAs)?
PFAs are former foster youth who work for Children and Family Services (CFS) to assist current foster youth/young adults ages 16 to 21. PFAs provide ILP services and support while assisting foster youth/young adults with their transition to becoming self-sufficient adults.
