ILP Service Delivery
- Every six-month reporting period the state pulls service information for youth within the ILP age range. Every youth who has an open case with the department has to have at least on service documented with that 6 month period.
- CFS ILP Participation of Youth in Placement
- The CFS ILP 2 can be used to gather information from the foster parent to enter into CMS as a Deliverable. There are helpful definitions with this form that will help you make sure the deliverable counts..
- Regional ILP Clerks and Peer and Family Assistants make sure ILP Events and workshops are entered into CMS as a deliverable (Another great reason for your youth to participate).

Seniors on Your Caseload
Seniors who are graduating will get a graduation incentive of approximately $200. For this incentive to be submitted for we will need the following:
- Seniors on your caseload will be tracked by the regions Education Liaison they will update us on who is on track to graduate and who may not
- If your senior is going to graduate, we will need to complete the High School Graduation Verification Form.
- We will also need to complete a W-9 with their youth.
- Official Transcripts, copy of diploma, and or a letter from the school explaining that the youth will graduate because of a specific plan created by counselor and student

Starting ILP Services for Your Client
- At 15 ½ a Transitional Independent Living Plan and Agreement (TILP) is required for your youth.
- This TILP will go to the ILP clerk for your region who will file the TILP and create a welcome letter to be sent to the youth. The clerk will also add the youth to the flier mail out distribution list that is used to get fliers for events and benefits to the youth via mail.
